How MSSP works

This voluntary program helps prevent unnecessary or inappropriate placement of elderly individuals in nursing homes, and is free to enrolled clients.

MSSP focuses on encouraging frail, elderly Contra Costa County residents to make healthy lifestyle choices regarding their social, health and housing needs.

The care team
A social worker and a public health nurse team up to provide clients with the most appropriate care available.

Creating a support plan
The MSSP enrollment process begins with a comprehensive lifestyle evaluation. After assessing the senior's needs, the care team works closely with the individual to determine what they want for themselves. The team provides information and counseling to the client and family and helps identify and coordinate services to which the senior is entitled.

Under the umbrella of MSSP, the care team is able to coordinate a broad range of at-home services. The care plan can include ongoing support from family and friends and/or a combination of existing local services. If these resources are not enough, the care team can help obtain and coordinate supplemental services for the client. MSSP supports families and primary caregivers of frail seniors by providing consultation and respite from daily responsibilities.

Coordinating a continuum of care
MSSP addresses the immediate needs of the frail senior as well as possible future needs. The MSSP care team works to ensure the older adult is benefiting from the additional support services.
