Who's Eligible for MSSP?

Any Contra-Costa County resident aged 65 years or older who:

	Receives Medi-Cal benefits
	Is in frail health
	Is at risk of entering a nursing home
	Needs ongoing case management services
	Voluntarily accepts MSSP services
