How linkages works

This program serves frail elderly people and other adults with disabilities by connecting them with support services in the community. Dependent adults who might unnecessarily be confined to nursing homes can find valuable resources that address their well-being and their desire to live at home. Even those who may not be eligible for other comprehensive services may still be entitled to assistance from Contra Costa County's Linkages Program.
