Who's eligible?

Frail elderly people and adults age 18 years or older, who have at least one of the following:

	Functional impairments that interfere with daily life
	Emotional and/or cognitive impairment rendering the adult unable to manage his/her own affairs
	A need for case management services

Eligibility is established with an initial screening and assessment. People who qualify and choose to participate benefit from a personal care plan that draws from a broad range of County services. Our community-based program links clients with transportation, meals and housing, money management, in-home support and adult day centers to alleviate stress for clients and their families.
